Each Department of Medicine houseofficer is given a Procedure Book at the beginning of
their training. All procedures performed by the houseofficer are to be logged in to the
procedure book except placement of peripheral short intravenous catheters.

Residents should serve as supervisors and teachers of procedures, once they are qualified
to do a certain procedure (DOM office has ABIM guidelines). It is important that all of our
houseofficers be given the opportunity to become qualified. The priority must be given to
residents, as they must get these procedures under their belts. Then, the interns must be
trained, and then, at the discretion of the resident, the student (especially subinterns)
may be given an opportunity. It is vitally important that each procedure is documented in
the resident's or intern's private procedure log. Non-medicine interns who are rotating on
a medicine service are given equal opportunity for procedures on patients assigned to their
team.

A procedure note must be written in the patient's chart immediately after the procedure is
completed. The note should be brief and must include the indications, that consent was
obtained, the basic procedure followed, whether or not there were complications, and the
names of all operators, including the staff attending (whether or not they were present).
